The VNA accepts Medicare (traditional and HMO), Medicaid, Worker’s Compensation, Veteran’s Administration, private insurance, and self-pay. For those who lack a third-party fee source, the VNA has a sliding scale fee for qualified individuals.

To arrange for service, simply fill out the online referral form below,

call our central number 1-330-782-5606 to speak with Jan Sherman, R.N, our Intake Coordinator, or a nursing supervisor. Services can be requested by a patient, family member, friend, or health care provider. We will contact your physician for authorization of services, verify insurance coverage, and arrange for the first home visit. At our first home visit, a nurse will consult with you, your family, and health care providers to develop a personalized treatment plan.
We will call you to confirm receipt of your E-mail referral within 1 working day during normal business hours.
