Choice

You, as a patient and family member, have the RIGHT to choose your home care provider. It is the law. Hospitals are required to provide a list of all Home Health Agencies serving the area where a patient resides.

The hospital has a responsibility to ensure freedom of choice to patients and provide information about the availability of home care. Hospitals must maintain a complete and accurate file on community based services and facilities, and inform the patient and family about their freedom of choice regarding providers.

Your care will not be jeopardized if you choose an agency other than recommended to you. Although many hospitals own home care agencies, they are separate entities with their own staffs.
